Clallam Mosaic strives to provide opportunities for people with intellectual and developmental disabilities and their families to access educational resources to:

Clallam Mosaic (Mosaic) provides Day Programs, Community Engagement (CE) services, client advocacy and information to individuals with special needs, their families and caregivers. In addition, Clallam Mosaic is the host for the Clallam County Parent to Parent chapter.

 

Mosaic services are available to individuals of all ages, races and gender.

 

Mosaic has created policies and procedures in order to maintain safety, insure consistent practices, afford equal opportunity to access of programs, maintain compliance with all applicable state, federal and local regulations, and insure the best possible experience for all served.

 

Mosaic does not provide transportation to and from Day Programs.

 

Transportation may be provided as part of CE services, field trips and/or other special activities.


Participant Code of Conduct for In-person Day Programs and Activities

For the health, safety and enjoyment of all Clallam Mosaic participants, the following rules are to be followed:

  • Have Fun!
  • Respect each other
  • Don’t hurt people’s feelings
  • Listen without interrupting when others are talking
  • Honor each other’s boundaries (physical space bubbles)
  • Be responsible for your own belongings
  • Do not touch or take things that do not belong to you
  • NO intimate displays of affection (This means no kissing or cuddling – it makes others feel uncomfortable & is not appropriate in public settings.)
  • Leaving Class: If you need to leave activities for ANY reason, you MUST get a hall pass from an instructor.
  • Cell phones: Turn volume to vibrate during classes & activities
  • All Electronic Devices: Laptops, iPads, cell phones, tablets and other screened devices may be used before/after class and during lunch (device use during class will be rare, and ONLY with permission from the instructor)
  • Don’t come to class if you are sick or unable to participate
  • Return your completed registration paperwork no later than the second week of each term

The Clallam Mosaic Code of Conduct is in place to make sure that our participants have the best learning environment possible. These guidelines allow our classes to run smoothly, so there will be consequences when not followed:

  • 1st time: Warning and possible notification of family/caregiver
  • 2nd time: Notification of family/caregiver plus a one-on-one conference between participant and Program Coordinator or Executive Director
  • 3rd time: Suspension from all programs for 1 week (no reimbursement)
  • 4th time: Suspension from all programs for the term (no reimbursement)

If your behavior poses a risk to the health, safety and/or enjoyment of those in the classroom, you will be asked to leave the room and your emergency contact will be notified.


Clallam Mosaic's Client Contract for Community Engagement


  • Commit to keeping appointments with your Community Guide – only canceling if you are ill. This means you will schedule other appointments and or activities around your Community Guide schedule.
  • Give 24-hour notice if you need to reschedule your appointment with your Community Guide. (Unless you become suddenly ill).
  • If you develop a pattern of missing appointments your Community Guide Services through Clallam Mosaic may be terminated.
  • Work with your Community Guide in creating a detailed plan to reach your goal as outlined in your Person-Centered Service Plan with DDA.
  • Sometimes setting and reaching goals can stretch us beyond our comfort zones. Please be willing to stretch your comfort zone and trust that your guide is there to support you. It will be well worth your effort!!
  • Refrain from using your cell phone or other electronic devices while with your Community Guide – unless using it for something related to reaching your goal.
  • Please only call your Community Guide during regular business hours. (Monday through Friday 9:00 am to 5:00 pm) Unless calling to reschedule due to illness.

Mosaic collects personal information from clients for a number of reasons, including:

  • To understand client needs and determine appropriate service provision
  • To report to funders and supporters
  • To appropriately advocate for clients
  • To improve services
